Take 3 to 5 grams 4 times per day for the first 5 days and then 3 to 5 grams once or twice per day thereafter (best to take 2 on training days and 1 on non-training days). Take it with a protein plus carb (at least 50 grams of carbs unless dieting) shake ideally. If not, take it just before a high-carb meal with water or juice (or any other drink with sugar).
Most experts now agree that there is no need to cycle off creatine or drink huge amounts of water, etc...
